Admissions Application Process  

The Fletcher School Admissions process is designed to educate our prospective parents and their child(ren) about our school and program. In addition, it allows us the opportunity to get to know your child and family. Based upon the information we receive with an application and through our admissions process, we are able to determine if our program is appropriate for your child. Fletcher requests that your child's psychological and educational evaluation include the WISC-III or WISC-IV and be current within a two-year time frame. If your child has an attention decifit disorder, we request that the neurological evaluation and diagnosis be no more than six months old.

The following checklist has been prepared to help you understand the steps necessary to be considered for admission. The application deadline is March 1, 2008.

Application completed by parent with a recent picture of your child and $100 non-refundable fee. Click to download.

Psychological and Educational Evaluation report completed by a licensed psychologist within the last 2 years. Please do not send originals. This information will not be returned. Click to download.

ADHD Evaluation Report from a physician, if applicable. The ADHD diagnosis cannot be more than six months old.

Student Health Record and Immunization Form completed by your child’s physician and mailed or faxed directly to The Fletcher School. A fax of this form is acceptable.   FAX 704-364-2978

Medical Report to be signed by parent and completed by physician IF your child has a medical condition that requires medication on an ongoing basis including ADHD, allergies or other conditions which would limit participation in activities.   Click to download.

Consent Form for Release of Information to The Fletcher School signed by parent to be copied and sent to all professionals who have information on your child. Please provide our school with one signed copy to cover all contacts with professionals.

Evaluations and/or Reports from outside services including Counseling, Neurological Evaluation, Occupational Therapy, Physical Therapy, Speech and Language Therapy, Vision Therapy, Educational Tutor and/or Consultant.

Teacher Recommendation from the school your child is presently attending. This form should be mailed directly from the school. If your child has more that one teacher, as in Middle or Upper School, please duplicate this form for these instructors. Click to download.

Recommendations from a counselor, resource teacher, tutor, or an administrator outside your child’s classroom who knows your child well. This form should be mailed directly from the school.

Copy of School Records including any current testing done by the school, exceptional children’s information, current report card(s), progress reports and any other information that you feel is helpful in describing your child.

Transcripts Required from middle and high school courses for students applying to
grades 7-12.

Placement Assessment at The Fletcher School. A $150 fee for this assessment is due the morning of the evaluation. Please pay by cash or check at the Front Desk.

Classroom Visit at The Fletcher School. Your child will experience a regular school day in their current grade placement one to two weeks after the assessment.

Grades 1-8, Attend  Fletcher School Summer Program July 9-27, 2007.
